Fix: Invoice Print

Multiple bug fixes are included in this release relative to Invoices.

Invoice Total was not calculating correctly when the invoice has commission items applied. This has been fixed. If there are multiple commission rates the invoice will display a commission item for each.
The credit memo date displayed on the invoice was the create date and not the applied date. This has been fixed.
